Web programing problem

The part I need help with is part 3, but will include what I have already if it will make it easier.

Part 1 was completed, part 2 however was not, what I need is part 3, if you need to do part 2 to complete part 3, that is fine, if you can use part 1 then it is there, if not then ignore it.
